Failed to send NIOS II request: Operation timed out

Hi everyone, I've recently purchased a new BladeRF due to a power issue with the old one. I am running into an issue trying to create the AGC tables, i've posted the error below. I've tried it with the most up to date firmware, fpga and libbladerf, i've also tried a combination of older firmwares and the corresponding FPGA file. The newer firmware/fpga causes it to time out quicker than the older ones. I would really appreciate any help on this. The only posts I can find related say to revert back to FPGA v11 but this makes no difference for me. I've also tried it on different versions of Ubuntu and on the raspberry Pi, same issue no matter what the OS version. I didn't have this problem on the old BladeRF so trying to work out what could be different in the space of a few weeks! Also tried different USB cables and ports.

Update: this combination of Firmware/FPGA works -

Firmware 2018-06-28 - v2.2.0
FPGA 2018-09-12 - v0.8.0
