bladeRF xA4 USB 3.0 Reconnect Causes FPGA Load Failure

Hi everyone,
I'm using a bladeRF xA4 connected through USB 3.0 on a Windows 11 system. The device works normally after a fresh boot, and the FPGA image loads successfully.
However, if I disconnect the bladeRF and reconnect it without rebooting the PC, bladeRF-cli sometimes reports that the FPGA failed to load, even though the device is still detected by the system. Restarting the application does not help, but rebooting Windows fixes the issue temporarily.
Has anyone experienced FPGA load failures only after a USB reconnect cycle? If so, were you able to identify whether the problem was related to USB power management, driver state, or firmware configuration?
